Prior to your first appointment, we ask that you fill out one of the following cards, depending on your type of appointment. Please bring the card with you to your appointment.
In order to maintain our high level of client service, we suggest that you arrive on time for your appointment. Late appointments may be rescheduled at our discretion.
All cancellations must be received no later than 24 hours prior to your appointment. Cancellations received within 24 hours of your appointment time may be subject to a cancellation fee.
